Coordinated Care Model

Coordinated Care Model

We understand that a cancer diagnosis is confusing and can be overwhelming to patients and their family members. We are proud to have a robust support team to care for our patients during their cancer journey. Cancer center patients are assigned a Nurse Care Manager (NCM) who provides education, coordination and overall supportive care. The NCM is the patient’s direct line to their Medical Oncologist for questions or concerns. Moreover, our patients have access to an Oncology Patient Navigator who can provide day-to-day support for items such as assistance with transportation and medication co-pays, as well as nutritional and emotional support. Our Center’s Nurse Care Managers and Patient Navigators work in concert to coordinate our patients’ treatments, tests and appointments along with the education and information specific to their cancer type and stage. Other Bloch Cancer Center support team members include a Licensed Social Worker, Financial Resource Counselor, Clinical Pharmacist, Clinical Psychologist and Clinical Trial Nurse.

Our cancer center is adjacent to the Truman Medical Center Main Hospital in the Hospital Hill Center Building.
